376 CARDIAC CONTRACTILITY MODULATION: A NEW THERAPY FOR HEART FAILURE

چکیده

Abstract Introduction Cardiac Contractility Modulation is a therapy for chronic Heart Failure patients with EF 25-45% in sinus rhythm QRS < 130 ms clinically symptomatic (NYHA Class > II) despite optimised medical therapy. It made possible by the implantation of OPTIMIZER SMART® device. The CCM works delivering non-excitatory high-energy bipolar signal, synchronised local electrical activity (or cooperating another pre-existing CIED), absolute ventricular refractory period, means two actively fixating leads, positioned interventricular septum and spaced at least 1-2 cm apart. Purpose Assessment feedback process dilated cardiomyopathy. Educational update health professionals. Patient education. Method A Pubmed search correlated clinical data field experience was performed. Results Patients severe Dilated Cardiomyopathy already on mechanism action demonstrated remodelling left ventricle so-called 'metabolic' In therapy, stimulation delivered to heart muscle during period. this phase, signals activate mobilisation calcium ions cardiomyocytes. applied period cause an increase cytosolic systole, resulting improved cardiac contraction without oxygen consumption. Conclusions has shown improve contractility performance improving quality life undergoing treatment reducing symptoms fatigue dyspnoea; it reduced number hospitalisations moderate failure. Healthcare professionals recognise typical ECG patient implanted device shows 'spike' cycle: 'R wave' complex being alarmed. been trained be autonomous charging. recharged externally weekly rechargeable battery through mini induction charger, itself.

Modulation of cardiac contractility. A potential treatment of heart failure?

During the last 10 years, electrical treatment of heart failure (HF) has become more and more popular. Cardiac resynchronization therapy (CRT), alone or combined with defibrillation, has been confirmed to be effective in patients with moderate or severe HF and ventricular dyssynchrony, manifest as a QRS duration .120 ms, though its effectiveness has not been confirmed in patients presenting wit...

Cardiac contractility modulation in patients with advanced heart failure.

Cardiac contractility modulation (CCM) is a novel device-based therapy for heart failure that involves applying electrical signals during the absolute refractory period of the myocardial action potential. This therapy has been shown to augment the strength of left ventricular contraction independent of myocardial oxygen consumption in animal models as well as human studies of patients with hear...

Cardiac contractility modulation by non-excitatory electrical currents. The new frontier for electrical therapy of heart failure.

Heart failure (HF) may complicate ischemic heart disease in both its acute and chronic manifestations, representing a prevalent health problem throughout the world. Development of therapies to improve heart function, relieve symptoms, reduce hospitalizations and improve survival is a high priority in cardiovascular medicine. The available pharmacological strategies, including angiotensin-conver...

SERCA2a: a prime target for modulation of cardiac contractility during heart failure

Heart failure is one of the leading causes of sudden death in developed countries. While current therapies are mostly aimed at mitigating associated symptoms, novel therapies targeting the subcellular mechanisms underlying heart failure are emerging. Failing hearts are characterized by reduced contractile properties caused by impaired Ca(2+) cycling between the sarcoplasm and sarcoplasmic retic...

Heart failure monitoring with a cardiac resynchronization therapy device-based cardiac contractility sensor: a case series

INTRODUCTION The SonR signal has been shown to reflect cardiac contractility. It is recorded with an atrial lead connected to a cardiac resynchronization therapy defibrillator. For the first time, clinical evidence on the use of the SonR signal in the monitoring of the clinical status of heart failure patients implanted with cardiac resynchronization therapy defibrillator are presented through ...
